Ask your friend to mention your referral code/name while booking appointment
After your friend completes his/her first appointment, you will get a $25 account credit

Terms and conditions
A 'Referral' is someone who is not an existing patient in our practice.
A family member of an existing patient or a member of the same household does not qualify for this program.
The referral must book an appointment directly with Aviva Dental Care.
The referral registering via third-party websites do not qualify for this program.
The referral must reference your name / referral code at the time of registration.
Once the referral completes his/her first comprehensive exam, full mouth x-rays, and clearning appointment, the rewards will be credited to your Aviva Dental Care patient accounts.
The referral credit cannot be combined with any other offer or dental plans and has no cash value.
The referral credit cannot be used for the initial visit, co-pay, co-insurance, deductible or emergency treatments.
The referral credit can be applied to any future dental service or procedures at Aviva Dental Care.
The rewards are non-transferrable. Must be 18 years or older to receive the rewards.
Federal law prohibits Aviva Dental Care from offering this promotion to anyone who is receiving services as beneficiaries under Medicare or state health programs.
The refer-a-friend program ends on January 31, 2024.
To qualify for rewards, the referrals must complete their first appointment before the program ends.
